I just added an Action but do not see it in my app.

Mosy likely, you did not "Save" the Action.

Fortunately, Actions are automatically saved in the Library. To add the new Action back to your App,

• tap "+ Add From Library", 
• sort the list by "Last Modified", which will move new Actions to the top
• identify your new Action and add it to your app
• Save it
